Use of
Chaperones
It is the policy of
this practice to make available a
chaperone should this be necessary. If a
chaperone is unavailable you may be
asked to return to the surgery at a time
when a chaperone will be available.

Can Help Us to Help You
Please let us know if you have a change
of address, telephone number or personal
details. This will help to ensure we can
contact you when needed. If you are a
carer please let us know as we may have
useful information for you.
We can only register patients in our
practice area. Please note that if you
move out of the area you will be
required to register with another
practice in your new area. This enables
us to register new patients who move
near to the health centre. Our reception
staff can offer advice on registering
with a new GP.
Teaching and Training at the Practice
This is a training
Practice.
We regularly have medical
students on placement with us. If you do
not wish the student to be present
during your consultation please let our
reception staff know.
Confidentiality
We need to hold
personal information about you on our
Computer system and in paper records to
help us to look after your health needs
and these records are confidential. We
comply with the Data Protection Act,
1998, which lays down legal requirements
for computer users such as ourselves.
We only ever use or pass on information
about you if people have a genuine need
for it e.g. in making clinical
referrals. Anyone who receives
information from us is also under a
legal duty to keep it confidential.
Sometimes the law requires us to pass on
information (e.g. to notify a birth or
when we encounter infectious diseases
that may endanger the safety of others).
In other circumstances you may be
required to give written consent before
information is released – such as for
medical reports for insurance,
solicitors etc.
To ensure your privacy, we will not
disclose information over the telephone
or fax unless we are sure that we are
talking to you. Information will not be
disclosed to family, friends, or spouses
unless we have prior written consent,
and we do not leave messages with
others.
You have a right to see your records if
you wish. Please ask at reception if you
would like further details. In some
circumstances a fee may be payable. The
Practice complies with the Freedom of
Information Act, 2000.
