This form is to allow you to ask advice and information if you have been unable to confirm details of an appointment.

  • A hospital outpatient appointment to see a specialist.
  • An appointment for outpatient investigations

We suggest you contact the hospital or service you were referred to directly first. See the ‘Contact Details for Hospitals’ triage for details of local hospitals and other services we commonly refer to.

If you have already been unsuccessful, we will provide you with the relevant information to chase up your referral or see what else we can do.

‘If this referral is regarding an urgent suspected cancer referral known also as a ‘Two Week Wait Referral’, please contact the practice via telephone on 02076196670 and select option 4 to speak with our medical secretary.’
